Intake Evaluation

This is the initial 50-minute appointment for new clients.   The fee for the initial appointment is $160.00.

After payment, a link will be provided for you to choose your appointment time.

Monthly Concierge Psychiatric Medication Support Services

This is for people who have completed their intake evaluation.  The monthly fee is $99.  Services include monthly 20-minute follow-up appointments, refills, same day responses to questions, laboratory monitoring, collaboration, and as-needed follow-up appointments.

After payment, a link will be provided for you to choose your appointment time.

Schedule Appointment

You may self-schedule using this brief form. The information provided will give us an update on how well a person is doing with their treatment journey.

After completing there will be a link for you to choose your appointment time.

Refill Request

Refills are processed more quickly when the pharmacy faxes the request to us at 213.927.3654.  However, you may also use this form to request refills.

Cancel Services

If for any reason you are not satisfied with our services to please login to your PayPal account and cancel.  You may also email us and we will cancel for you.
